Horizontal gaze nystagmus is found in individuals with diseases of the brain, particularly lesions.

Gaze nystagmus can be found in healthy individuals as well. This is known as ‘end-point nystagmus.’ It is less intense than in individuals suffering from disease, but could certainly be sufficient to cause a person to ‘fail’ the horizontal gaze nystagmus test proffered by a law enforcement officer.
